When must morbid obesity be operated on? From a BMI of 40kg/m2 or 35kg/m2 with associated comorbidities.

Which examinations are necessary before the operation or for health insurance approval? An internal-endocrinological examination, a clinical psychological assessment, a professional diet consultation, as well as an upper abdominal sonography (gallstones?) and a gastroscopy (hiatal hernia? biopsies).
